Our center has compiled 24 collections of research literature on agarwood. In terms of the completeness of the evidence chain, sleep (sedation and calming) ranks first—a total of 45 papers from 1998 to 2026, with verified evidence at all six levels. Other directions have only animal experiments or only in vitro data; only this one has reached Phase III human clinical trials.

1. Six Levels of Evidence, Layer by Layer

The first level: traditional records. The Compendium of Materia Medica states that agarwood "clears the human spirit," and successive materia medica works contain statements such as "benefits qi and harmonizes the spirit, treats insufficiency of the heart spirit." This is not modern packaging but the accumulation of more than a thousand years of use.

The second level: animal pharmacodynamics. Multiple pentobarbital sodium synergy experiments have provided reproducible quantitative data: agarwood incense sticks at 0.125 g per animal, agarwood powder at 0.1 g per animal, and volatile oil at 1 μL per animal all prolonged sleep duration (Chinese Pharmaceutical Journal 2022; Modern Chinese Medicine 2022). In PCPA-induced insomnia model rats, neurotransmitter changes were consistent in direction: GABA increased, the GABA/Glu ratio increased, and 5-HT and adenosine increased.

The third level: molecular mechanism. Agarwood extract enhances GABAA receptor function and promotes chloride ion influx, and this effect can be blocked by the receptor antagonist flumazenil—indicating that it shares a molecular target with diazepam, but its pattern of effect on receptor gene expression differs from that of diazepam, providing a mechanistic explanation for the absence of tolerance.

The fourth level: human randomized controlled trials. 120 patients with insomnia disorder, randomized, with a glycerol active placebo control, for 3 consecutive weeks, assessed using five scales: PSQI, PSAS, ISI, GAD-7, and PHQ-9 (Journal of Stroke and Nervous Diseases 2019). This is the largest RCT of agarwood incense therapy in China.

The fifth and sixth levels: new drug translation. The anti-anxiety drug buagafuran, derived from the agarwood furan structure, has completed Phase II clinical trials (144 patients, 60/120 mg·day⁻¹, 6 weeks, effective with "no tolerance observed"), and a Phase III trial of 410 patients is ongoing (active comparator buspirone). That a component of a traditional medicinal material can enter the modern new drug pipeline is itself a hard indicator.

2. The Most Valuable Theme: No Tolerance

The biggest problem with sleeping pills is tolerance and dependence. In research in this direction, "no tolerance" runs through three levels: no tolerance after 14 days of dosing in animals, a different receptor effect from diazepam mechanistically, and no tolerance observed in human Phase II trials. The three levels corroborate one another and constitute the core selling point that distinguishes agarwood from benzodiazepines—and the reason we continue to follow this direction.

3. Shortcomings That Must Also Be Stated Clearly

The compilation principle of the collection is "refuse to fabricate," and several gaps are honestly declared: polysomnography (PSG) studies, pharmacokinetic studies, and long-term toxicology studies are currently all blank. Claims circulating online such as "regulates brain waves" and "enhances alpha waves by 15%–20%" were checked and found to be entirely without sources; do not cite them. In addition, some studies have reported that agarwood incense has "no significant effect on mood"—this negative result also deserves respect; aiding sleep and improving mood are two different things.

Agarwood for Sleep

From the Compendium of Materia Medica's 'clears the human spirit' to a 120-patient randomized controlled trial and buagafuran now in Phase III—sedation and calming is the most complete evidence chain of all agarwood research directions. This article breaks it down layer by layer.
