"Agarwood Boosts Immunity"? Half Wrong: The Bidirectional Truth in 48 Papers

Of ten articles about agarwood on the market, nine claim it "boosts immunity." The conclusion of the verified Compendium of Research Literature on Agarwood and Immunity is: that statement is half wrong. Agarwood's effect on the immune system is not unidirectional but a classic case of bidirectional regulation—different extracts and different monomers from the same medicinal material can exert completely opposite effects on different immune cells.

1. Evidence on the enhancing side

In mice with cyclophosphamide-induced immunosuppression, both whole-body agarwood ethanol extract (Shizhen Guoyi Guoyao 2020) and agarwood pills (Biotechnol Bioprocess Eng 2023) significantly increased thymus/spleen indices, lymphocyte proliferation, antibody production, and NK cell activity; agarwood methanol extract activated macrophages and promoted phagocytosis via the MAPK pathway (RAW264.7 experiments). Such data support "restoring toward normal under immunosuppressed conditions."

2. Evidence on the suppressing side

The other side is equally solid: the sesquiterpene derivative HHX-5 simultaneously inhibits macrophages, neutrophils, T cells, and B cells, with the mechanism involving STAT1/4/5/6 phosphorylation (Eur J Pharmacol 2016); the chromone derivative GYF-21 inhibits B cells via the BAFF pathway (Int Immunopharmacol 2019); multiple chromone components inhibit LPS-induced release of inflammatory cytokines. In the anti-allergy direction, there are also rigorous studies reporting IC50 values and negative results (J Pharmacopuncture 2016; Aquimavitalin in Int J Mol Sci 2016 provided a therapeutic index).

3. Why this matters to you

Two practical judgments. First, "immune regulation + conditions of action" is the accurate formulation—people with autoimmune issues should be wary of promotional claims in the pro-inflammatory direction, and expectations among immunocompromised populations should also be tempered. Second, the compendium classifies evidence into four levels: original research (Level A/B) can be cited, review paraphrases (Level C) must be flagged, and patent claims and online popular-science content (Level D) are never used as evidence—the widely circulated "98% effective rate" belongs to Level D; steer clear when you see it.
