Agarwood and Cancer: Five Standout Findings from 57 Papers, and One Sobering Caveat

First, the sobering caveat: among the 57 papers in the "Compendium of Agarwood and Cancer Research Literature," not a single human anti-tumor clinical trial is included—not even a case observation. All evidence remains at the cellular and animal level. This article presents the five most striking findings together with this reality; both halves are facts.
I. Five Landmark Findings
① Ferroptosis against gastric cancer (Beijing University of Chinese Medicine, Chin J Nat Med 2025). The petroleum ether extract of agarwood showed IC50 values as low as 2.37–2.89 μg/mL against three gastric cancer cell lines, with mechanisms spanning three pathways: the DNA damage-p21-CDK4 axis, HO-1-mediated ferroptosis, and BAX/BAK apoptosis; it was also validated in nude mice with no significant toxicity. Ferroptosis is one of the hottest directions in current tumor pharmacology, and this study directly connects agarwood to it.
② Anti-cancer molecules from Aquilaria sinensis flowers (Kunming Institute of Botany, CAS, Nat Prod Bioprospect 2022). Cucurbitacin E had an IC50 of only 0.002 μM against paclitaxel-resistant lung adenocarcinoma cells A549/Taxol, compared with 1.80 μM for paclitaxel—approximately 900-fold stronger activity—offering a new approach to overcoming paclitaxel resistance. This finding was reported by national media including Xinhua News Agency and represents the discovery with the greatest social impact in this direction.
③ Precision targeting of KRAS-mutant lung cancer (Acta Pharmaceutica Sinica 2024). Compound 8 was identified from 19 2-(2-phenylethyl)chromones and showed the best activity across three KRAS-mutant cell lines, inhibiting the c-Met pathway, with in vivo validation completed using a zebrafish model. KRAS is widely recognized as one of the most difficult targets to drug.
④ The most complete druggability assessment (BMC Complement Altern Med 2016). In the HCT-116 nude mouse model, agarwood oil at 200 mg/kg significantly reduced tumor size, with necrosis and disappearance of neovasculature observed in tumor tissue, and an LD50 greater than 2000 mg/kg—the only study in the compendium that addresses both "safety" and "efficacy."
⑤ Best selectivity (Molecules 2022). Novel cucurbitacin-type compounds were effective against most cancer cells at 5 μM and showed higher sensitivity toward cancer cells than normal cells—selectivity is the most core metric for evaluating anti-cancer candidates.
II. An Industry Insight
Many of the active substances in the anti-cancer direction come from non-heartwood parts: cucurbitacins from Aquilaria sinensis flowers, patented components from Aquilaria sinensis fruit, and flavonoids from agarwood leaves. This is one of the strongest scientific bases for "whole-plant utilization of agarwood"—leaves, flowers, and fruit are not waste material but another treasure trove.
III. Two Things That Must Be Made Clear
First, dose realism. The in vitro IC50 of some chromone components is at the mg/mL level, whereas the pharmacopoeia specifies only 2–5 g of agarwood in decoction; achieving such concentrations in the human body is nearly impossible. In the same review, the volatile oil IC50 was only 11 μg/mL—nearly a 200-fold difference among different components, so units must be verified item by item when citing. Second, in vitro inhibition of cancer cells ≠ ability to treat cancer; in between lies the long road of Phase I–III clinical trials, dose determination, and long-term safety evaluation. Any promotion that packages agarwood as an alternative cancer therapy has no scientific basis—we will never cross this red line in our own promotional content either.

常见问题
Agarwood and Cancer是什么?
Cucurbitacin with ~900-fold stronger activity against paclitaxel-resistant cells, and gastric cancer research linking to ferroptosis—lab results are striking, but none of the 57 papers includes human clinical trials.
